Identical (Monozygotic) Twins
Identical twins form when a single fertilized egg splits into two embryos. The timing of the split determines the placenta and sac configuration:
- Days 1-3: Two placentas, two sacs (dichorionic-diamniotic) - safest configuration
- Days 4-8: One placenta, two sacs (monochorionic-diamniotic) - most common for identical twins
- Days 8-12: One placenta, one sac (monochorionic-monoamniotic) - requires very close monitoring
- After day 12: Conjoined twins (extremely rare)
Differences in Medical Monitoring
Fraternal twins with separate placentas typically need ultrasounds every 4 weeks. Identical twins sharing a placenta require monitoring every 2 weeks due to the risk of twin-to-twin transfusion syndrome (TTTS), a condition where blood flow between the twins becomes unbalanced.
