Identical vs Fraternal Twins: What Are the Differences for Pregnancy?
Identical and fraternal describe zygosity; pregnancy monitoring is driven mainly by placentas and sacs. Learn the difference and why it matters.
The essential distinction
Identical versus fraternal describes zygosity—how the twins formed. Dichorionic versus monochorionic describes whether the babies have separate or shared outer membranes and placental circulation. Diamniotic versus monoamniotic describes whether they have separate or shared amniotic sacs.
For pregnancy care, chorionicity and amnionicity often matter more than zygosity. NICE explicitly states that the risks associated with twin pregnancy are determined by chorionicity, not zygosity.
Fraternal twins: two ova
Dizygotic twins form when two ova are fertilized. Genetically, they are like other full siblings and can be the same sex or different sexes.
They are usually dichorionic diamniotic: each fetus has its own chorion and amniotic sac. Two placentas can sit next to each other and appear fused, so the number of visible placental masses is not a reliable lay test of zygosity.
Family history, maternal age and population context mainly relate to spontaneous dizygotic twinning. The twin probability calculator therefore applies those weights only to its fraternal component.
Identical twins: one embryo splits
Monozygotic twins form from one fertilized ovum. Depending on early development, they may be:
- dichorionic diamniotic;
- monochorionic diamniotic;
- monochorionic monoamniotic.
This is why “identical” does not automatically mean “shared placenta,” and why zygosity alone is not enough to plan monitoring.
How the pregnancy type is established
NICE recommends a first-trimester ultrasound to estimate gestational age and determine chorionicity and amnionicity. Assessment uses features including placental masses, membrane thickness and the lambda or T-sign. If the type remains uncertain, the guideline recommends specialist review and management as monochorionic until proven otherwise.
Different-sex twins are dizygotic. Same-sex twins can be dizygotic or monozygotic, so appearance, sex and family history cannot always resolve zygosity.
Why monitoring schedules differ
Shared placental circulation creates complications that do not occur in the same way when placentas are separate. For uncomplicated dichorionic twins, NICE recommends growth-discordance monitoring from 24 weeks at intervals no longer than 28 days. For monochorionic twins, it recommends ultrasound monitoring every 14 days from 16 weeks for feto-fetal transfusion syndrome and other complications of monochorionicity.
Those intervals are guideline context, not a personal care plan. Findings, symptoms, local protocols and other complications can change the schedule.
What to ask at an appointment
Useful questions include:
- What are the chorionicity and amnionicity?
- How certain is that classification?
- What monitoring schedule applies to this pregnancy?
- Which symptoms should prompt urgent contact?
Use the clinical team’s documented ultrasound classification rather than an online description to understand an individual twin pregnancy.
